This form is designed to serve as a guide to assist you in preparing your reports to the Until There's A
Cure Foundation. The information listed here
is the minimum required. You may add additional information, if appropriate. You can print and fill out
this form or reproduce it using a word processor. If you prefer to use a word processor, you can open the
equivalent RTF file (assuming your browser is configured to
support RTF format documents) and work on it in your word processor, such as Microsoft Word.
Your final report is due to the Foundation by October 31 of the year following the grant award,
unless otherwise specified in your Grant Agreement (that is, by October 31, 2007 for the 2006 grant year;
by October 31, 2008 for the 2007 grant year). Failure to submit timely reports may be cause to terminate
the grant, may require that funds be returned to the Foundation, or may reflect negatively on requests
for future funding from the Foundation.
